When is the root canal treatment necessary?

A root canal treatment is required when the dental pulp (a nerve) present in a root canal is affected, as well as in the case of a tooth prosthetic procedure when the dental crown is significantly damaged.

A root canal treatment is required when inflammation develops inside the root. This inflammation might cause severe pain after contact with a cold drink or food and while chewing.

The aim of the endodontic treatment is to destroy the microorganisms causing inflammation and to protect other teeth and adjacent tissues from infection. If root canals are not treated, the tooth can be damaged irreversibly and might need to be extracted.

The process of a root canal treatment

A root canal treatment involves the treatment of root canals and the bone surrounding the apical part of the root. All tissues inside a tooth (known as the  pulp) need to be devitalised. The treatment is divided into two stages: the chemo-mechanical preparation of the canals and their filling.

The tooth is isolated by applying a protective layer of liquid rubber. Then, the number of canals and their length is determined and an X-ray is taken.

Later, the remainder of the tissues affected by the inflammation are removed, while the canals are widened and washed with a disinfectant solution.

When the canals have been properly cleaned and prepared, they are hermetically filled with a special substance and the shape of the tooth is restored with a filling or a crown. The tooth will then function just like a natural one.

Regular teeth fillings

A tooth filling procedure is a therapeutic dental treatment involving the removal of hard dental tissues affected by decay and caries and their restoration using fillings.

This procedure is required when decay (a hole) develops in a tooth. It takes about 6 months for a large hole to form from a primary tooth decay. Therefore, if you experience early symptoms like sensitivity or tooth pain, you should seek medical advice, as small holes are easier to treat than deep caries.

If a dental filling is delayed, the caries hole will become bigger, acute pain might develop and an infection can penetrate into the dental nerve and osseous tissues. A tooth and its adjacent tissues can suppurate, and food particles may get trapped in the hole. This may result in a tooth fracture, and an excessive delay may lead to the extraction of the damaged tooth.

After applying a local anaesthesia, the tissues affected by the caries are removed. Then, the cavity is disinfected. Depending on the depth of the damage, a lining can be applied (a lining is a substance that emits fluoride ions and hardens the floor of the cavity as well as having a positive effect on the dental pulp; it can allow the future endodontic treatment of dental root(s) to be avoided).

After the tooth preparation and cleaning of the caries, the tooth is restored by a filling substance. The colour and chewing surfaces of the restoration will be individually adjusted to the colour and functions of the tooth.

When a caries is removed using a small drill, the nerve endings are exposed and they are irritated by the filling material. Therefore, the sensitivity of the teeth is increased, which can last for one to several weeks. Pain stimulants should be avoided during this period, such as cold, hot and hard foods.
